Flow Cytometry Forms

In order for users to access services provided by the Flow Cytometry Core Facility, please review and complete the forms below, as appropriate.  Use of the Flow Cytometry Core Facility requires that all personnel be familiar with the policies and procedures.  All Principal Investigators are required to submit a current bio-hazard risk assessment form annually. In addition, each cell sort performed within the facility requires prior submission of a completed risk assessment form for all Arias and the MoFlo.
